Completing the Biometric Data Disclosure and Authorization Form

Keeping your organization compliant with biometric data regulations is important, especially when using the Citadel fingerprint scanning time clocks. To ensure compliance with biometric regulations, organizations using our time clocks are required to complete the Biometric Data Disclosure form. This form outlines the procedures and policies related to the collection and storage of biometric information. As an administrator, you will need to verify your company's adherence to these guidelines at least once every 30 days. This article will walk you through the steps to accurately fill out and sign the form in our system.

Complete the Biometric Data Disclosure:

1. Check the first box on the form in the Download, edit and print forms section.
2. Enter the initials displayed in the prompt above the Initials field exactly as they appear in the prompt.
3. Click the Download Forms button to download the Biometric Data Disclosure and Authorization form.
4. Obtain the necessary consent for your region and select the appropriate option in the Obtain employee written consent following required notice section.
5. Review the terms listed. 
6. At the bottom of the form, check the box stating to confirm 'I have read and accept the terms of this agreement'.
7. Enter the name displayed in the prompt above the Name field exactly as it appears in the prompt.
8. Click the ACCEPT button.
