The biometric compliance form is required to acknowledge and authorize the collection and use of biometric data, such as fingerprints. This form must be signed when setting up your account, periodically throughout the year, and when adding new employee biometric data. You can download the Citadel Biometric Data Disclosure and Authorization Agreement directly from your portal at any time. We have attached a PDF of the form below for your convenience.

- Log in to the Citadel web portal.
- Click on People under Organization in the side menu.
- Click the Download Compliance Form icon
in the top-right corner of the page .
